At Phoenix Art Museum, your satisfaction is our number one priority. If for any reason you are not completely satisfied with your purchase, please return it to us for an exchange or prompt refund of the merchandise price.
Ordering Instructions
How to submit Order Form
This form cannot be submitted online. You may fill out the form online, print it and either fax it or mail it to the Museum, or you may print a blank form and fill it out by hand and then submit it by fax or mail. We suggest that you keep a copy of the completed Order Form that you submit.

Member Discounts
If you are a current Member of Phoenix Art Museum, you are entitled to the Member's 10% discount on purchases (except for Gift Certificates and where noted in product descriptions). Please enter and subtract 10% of your total order amount where indicated at the bottom of the Order Form. Also, remember to provide your Phoenix Art Museum Membership number in the BILLING INFORMATION section of the Order Form for verification, or the discount will not be applied to your order. Refunds & Exchanges
If you are not completely satisfied with your purchase, please return it for an exchange or a refund of the merchandise price. For gift returns, refunds will be applied to the purchaser's account. When exchanging merchandise, please include the appropriate shipping charges for your new order.
Gift Orders
If you are ordering a gift and want it to be shipped directly to the recipient, enter the recipient's information in the SHIP TO section of the Order Form, and any message you would like on an enclosed gift card. If the gift purchase is returned for a refund, the refund will be applied to your account as the purchaser.
Gift Certificates
Gift Certificates for any amount may be purchased with this Order Form and they can be sent directly to the recipient. Member discounts are not applicable when purchasing a Gift Certificate. Gift Certificates are shipped via U.S. Certified Mail. If you are ordering a Gift Certificate in addition to other merchandise, please add the shipping charge for the Gift Certificate to the shipping charge applicable for the rest of your order.
Shipping
All in-stock items usually are shipped within 48 business hours after the order is received, via regular UPS ground. Orders to be shipped to P.O. boxes will ship via U.S. Mail. Orders with international destinations are sent via surface mail. Gift Certificates are sent via U.S. Certified Mail. Domestic delivery time is up to 7 days; Canadian delivery time is up to 10 days; and international delivery time is 2-6 weeks. Shipping rates are based on total dollar amount of order and destination via standard delivery. International Orders
All prices shown are in U.S. dollars. Any applicable foreign duty charges and taxes are the purchaser's responsibility. Special shipping rates apply for deliveries to destinations outside of the United States.
Out of Stock
If an item is temporarily out of stock, you will be notified by e-mail with an estimated time of delivery. Please be sure to provide your e-mail address on the Order Form.
